Cara Cafe

  • A monthly meet – up for those living with dementia, their families and carers and community members who wish to support a dementia inclusive community in Ballyroan

 

  • To support older people to live in their own home with dignity and independence for as long as possible 
  • our service 
  • we facilitate supports on:

Housing, Health , Finance, Community , Technology, Climate, Energy.

  • Living well is a free, group self -management programme for adults 18 years and with long -term or chronic health condition.

Along – term health condition is one that can be treated and managed but usually not cured.

  • Examples include asthma ,  COPD , diabetes , heart , conditions , stroke , multiple , sclerosis , arthritis , inflammatory bowel disease , 

chronic pain and many more.

  • The Living well programme may also be a support for those with long- COVID.

 

  • A social prescriber can meet with you and support you to access local voluntary community and social enterprise organisations or services.
  • you can get a referral from any healthcare professional , such as your GP or public health nurse , or contact the service directly for more information :  info@ sdcpartnership.ie

 

  • MTRRs are a national free service that provide practical information and advice on assistive technologies to help people at different stages of dementia and memory loss.
  • Who can use this service ?
  • people with dementia , or other memory problems 
  • family and carers of people with dementia 
  • Healthcare professionals
